Toxic Discovery Survey
Toxic Discovery is conducting an informal survey to inquire on the numbers of individuals that have implants but cannot afford to have them removed.
Please forward to any and all individuals that you believe might fall into this category. Please have them contact us at the below email address with the following information:
1. Name
2. Address - City - State - Zip
3. Phone
4. E-mail Address
5. Short note on when you were implanted, Manufacturer of implants and what happened to you since being implanted. If you have a documented rupture, please alert us to that fact and what documents you have that would prove rupture. Also any health issue that you are having at this time.
