The SF-DCT continues to develop a list of physicians who have submitted acceptable affidavits or affirmative statements that show they used Dow Corning implants during certain years, in order to assist claimants who have been unable to establish the manufacturer of their implants.

If you submit medical records that show you had implant surgery performed by one of the physicians on this list, during the time frame(s) listed, you may submit these documents to the SF-DCT for consideration.
